IDDBA Food Safety Certification
Reimbursement Program (FSCRP)

(IDDBA Retail Members Only)

To promote food safety certification for members, the International Dairy-Deli-Bakery Association (IDDBA) has developed the Food Safety Certification Reimbursement Program. This IDDBA program will reimburse IDDBA retail members for their cost for food safety certification exams.

ELIGIBILITY

Employees of Current IDDBA Member Retail Companies

• Click here to check your member status.
• Store-level dairy, deli, and bakery managers and associates
• Passed certification exam
• Reimbursement has not been issued in the past at any store for that employee (Contact IDDBA at 608-310-5000 to verify previous reimbursement for employees.)

Exams

• Certified Professional Food Manager – Prometric (www.prometric.com/foodsafety/CPFM.htm)
• Food Safety Manager Certification or SuperSafeMark - National Registry of Food Safety Professionals or FMI (www.nrfsp.com)
• ServSafe® Food Protection Manager Certification - National Restaurant Association Educational Foundation (www.nraef.org)*
  *ServSafe is a registered trademark of the National Restaurant Association Educational Foundation.

Testing Periods and Application Deadlines

Incomplete applications will not be considered.

REIMBURSEMENT

• Only the cost of the exam (NOT training, materials, labor hours, meals, travel, salaries, etc.)
• Each member company is capped at a maximum reimbursement of $2,000 per fiscal year (July-June).
• Because funds are budgeted annually, available funds may be prorated among eligible applicants.
• IDDBA reserves the right to determine all reimbursements.
• Reimbursement will be in the form of a check to the IDDBA member.
• Please do not submit a reimbursement request of more than $2000 on one application.

Incomplete applications will not be considered.

HOW TO APPLY

Application

1. Download the application        
2. Open FSCRP application file in Microsoft Word
   Note: If you see any of these warnings, click "Enable". It might take a few seconds to load and be ready to use.

3. Complete all fields on the application.
4. Go to the Roster of Employees Form section for more information.
5. Use the Application Summary as a checklist after you have completed the rest of the application.
6. Save application (and Spreadsheet Roster Form) with your company name in the file name.

Roster of Employees

• You may use the Roster of Employees Form included in the application, however, if you are submitting a reimbursement request for more than 12 employees, please use the Spreadsheet Roster Form.
• The Spreadsheet Roster Form may also be used for fewer than 12 employees.
• Download the Spreadsheet Roster Form.

Required Attachments (Email, Fax, or Mail)*

1. Copy of the invoice for payment of exam for each employee
2. Copy of each employee’s certificate or a pass list from the testing company
3. A roster of employees – Roster of Employees Form or Spreadsheet Roster From or your own list with the following information:
   1. Employee name, title, department, and home address
   2. Store name, number where applicable, and address
   3. Date of training and date of testing
   *In an effort to reduce paper waste, we encourage applicants to email all application materials, including scanned or electronic copies of the above attachments.

After completing your application and roster of employees, please complete the Application Summary. This will help ensure your application is complete.
